Process and Requirements

Our state-licensed residential care facilities maintain the highest standards of compliance with Department of Behavioral Health and Developmental Services (DBHDS) regulations. See the requirements below for program and service qualification as well as our application process.

Admissions Process

Haven Residences Admissions Process

1. Initial Inquiry
The process begins with an exchange of information to determine if Haven Residences can meet the individual’s needs.

2. Referral Submission
Referrals may come from:

  • Parents or guardians

  • Social workers

  • Government officials

  • Private practitioners

  • Psychiatric facilities

  • Special education personnel

  • CSB/BHS representatives

All referrals should be directed to the Admissions Coordinator or designee.

3. First Contact & Preliminary Screening
A phone call is acceptable to start. However, the process will not move forward until the referral packet and required documentation are provided. During initial contact, the Admissions Coordinator or designee completes a preliminary screening form.

4. Request for Documentation
If both parties wish to proceed after the initial screening, the Admissions Coordinator or designee will send the Admission Screening Packet and request the following:

  • Current ISP

  • SIS

  • VIDES

  • Psychological Evaluation

  • Behavior Support Plan (if applicable)

  • Current medication list and medical protocols

  • Any additional documentation needed

5. Review and Follow-Up
Within three (3) days of receiving the requested documents, the Admissions Coordinator or designee will:

  • Provide next steps

  • Extend an offer for admission or

  • Request additional information if needed

Admissions Criteria

Admission Criteria – Haven Residences

To be considered for placement at Haven Residences, individuals must meet the following criteria:

    1. Primary Diagnosis
      The individual must have a primary diagnosis of an intellectual disability.

    2. Qualified Assessment
      The individual must be assessed by a QDDP or QMHP-A. The assessment must show a need for residential support that Haven Residences is able to provide. We do not accept individuals whose needs exceed our level of care.

    3. Basic Self-Care Ability
      The individual must be able to meet basic personal and self-care needs with support, training, or assistance.

    4. Medical Stability
      Individuals must be medically stable. If on-site medical treatment is required, admission may be considered only if there is space in a home equipped to provide those services.

    5. Age Requirement
      The individual must be at least 18 years old.

    6. Behavioral and Mental Health Considerations
      Individuals exhibiting self-injurious behavior, maladaptive behaviors, or mental health symptoms that could pose a risk to others, the program, or the community must be willing to participate in appropriate treatment services. Ongoing therapy or behavioral consultation may be required as a condition of admission.

    7. Health Screening
      A physical examination and tuberculosis test with negative results must be completed within 30 days prior to admission. Haven Residences may request updated health documentation before making a final admission decision.

    8. Program Agreement
      Individuals must agree to follow the rules, policies, and expectations of the program.

    9. Compatibility and Suitability
      Admission will not be offered to individuals who do not meet or agree to the above criteria, or whose needs or functioning levels are not compatible with those of current residents.

Exclusion Criteria

Non-Acceptance & Exclusion Criteria – Haven Residences

    1. Written Notice for Non-Acceptance
      If an individual is not accepted into the program, Haven Residences will provide a written explanation stating the reason(s) for the decision.

    2. Behavioral and Safety Exclusions
      Individuals will not be considered for admission if any of the following apply:

      • A recent and persistent history of illegal sexual behavior

      • A persistent history of fire setting

      • A recent and persistent criminal history involving felony convictions and incarceration

      • A recent and persistent history of substance abuse, or current active substance use

    3. Substance Abuse Consideration
      If appropriate, successful completion of an inpatient detoxification program may be required before admission can be considered.

    4. Mental Health Safety Requirement
      Individuals presenting active suicidal ideation with a current plan at the time of admission will not be accepted.

Continued Stay Criteria

Ongoing Eligibility and Participation – Haven Residences

  1. Continued Compliance
    Once admitted, individuals are expected to continue meeting all admission criteria throughout their stay.

  2. Reassessment of Needs
    If an individual’s needs change, Haven Residences will reassess their current level of functioning and support requirements to determine continued eligibility.

  3. Discharge or Termination
    If it is determined that an individual is unable or unwilling to meet the program’s admission or participation criteria, discharge or termination from the program may occur.

  4. Program Rules
    All individuals must agree to follow the rules and expectations of the program at all times.

Discharge Criteria

Discharge Criteria – Haven Residences

Haven Residences has established clear discharge criteria for individuals in its care. Discharge may occur based on any of the following:

  1. Voluntary Discharge
    The individual requests to leave the program.

  2. Needs Exceed Program Capacity
    The individual develops support needs that Haven Residences is not equipped to provide.

  3. Reduced Need for Support
    The individual has progressed to a level of independence where group home living is no longer appropriate.

  4. Loss of Eligibility
    The individual no longer meets the program’s eligibility or admission criteria.

  5. Failure to Follow Program Rules
    Discharge may occur when an individual poses an immediate or significant danger to themselves or others, or repeatedly violates program rules, including but not limited to:

    • Engaging in violent behavior that results in injury or threatens the safety of others

    • Repeated and willful non-compliance with behavior plans when behaviors or mental health symptoms present a risk to self, peers, staff, or the public

    • Interfering repeatedly with another individual’s program plan

    • Exhibiting disruptive behavior that violates the civil rights of others

    • Knowingly exposing others to life-threatening communicable diseases

    • Presenting with active mental health symptoms that require long-term inpatient hospitalization or repeated hospital stays

    • Engaging in behaviors that significantly infringe on the rights, health, safety, or mental well-being of themselves, other residents, staff, or the public

  6. Interference from External Parties
    Discharge may also occur when interference by parents, guardians, authorized representatives, family members, or others significantly disrupts program operations or management. This includes situations where the interference:

    • Prevents the proper implementation of the individual’s program plan

    • Hinders the achievement of established goals

    • Obstructs compliance with physician or licensed practitioner recommendations

The admission process begins with an online tour or placement request. After reviewing the individual’s needs, our team will schedule a consultation, verify insurance or waiver eligibility, and confirm home availability. Once approved, we develop a personalized care plan to ensure a smooth transition.
